Update on the use of lipid emulsions in local anesthetic systemic toxicity: a focus on differential efficacy and lipid emulsion as part of advanced cardiac life support.

چکیده
Local anesthetic systemic toxicity (LAST), including cardiovascular (CV) and central nervous system (CNS) complications, is a rare but devastating complication of regional anesthesia. Soon after the first successful use of cocaine as a local anesthetic (LA) in the late 19th century, there were case reports describing systemic toxicity and recommendations for its prevention. Arguably, LAST has been the major impetus for the development of newer (and safer) LA agents throughout the 20th century. Until the last decade, there were few options for treatment of LAST beyond a prolonged cardiopulmonary resuscitation (CPR). However, the recent successful use of intravenous lipid emulsions in severe LAST has dramatically changed the clinical management of this dreaded complication.

Reversal of Cardiovascular Toxicity in Severe Organophosphate Poisoning with 20% Intralipid Emulsion Therapy: Case Report and Review of Literature
Background: Cardiac toxicity is one of the life-threatening effects of severe organophosphate (OP) poisoning. We presented a patient with severe OP poisoning, in cardiovascular shock poorly responsive to conventional treatments, who could be resuscitated successfully with intravenous lipid emulsion (ILE) therapy. Case report: A 26-year-old female was admitted to our emergency department who had...
متن کاملLipid Emulsion for Local Anesthetic Systemic Toxicity
The accidental overdose of local anesthetics may prove fatal. The commonly used amide local anesthetics have varying adverse effects on the myocardium, and beyond a certain dose all are capable of causing death. Local anesthetics are the most frequently used drugs amongst anesthetists and although uncommon, local anaesthetic systemic toxicity accounts for a high proportion of mortality, with lo...

CLINICAL QUESTION Is intravenous lipid emulsion a safe and effective therapy for the reversal and treatment of local anesthetic toxicity? RESULTS Systematic reviews, human case reports, and experimental animal studies have demonstrated the efficacy of intravenous lipid emulsion therapy in successfully reversing cardiac arrhythmias, cardiac arrest, and cardiac collapse seen with severe systemi...
متن کاملLipid resuscitation for local anesthetic toxicity: is it really lifesaving?
PURPOSE OF REVIEW Laboratory studies and clinical reports have led to the acceptance of lipid emulsion as an effective treatment of local anesthetic-induced cardiac arrest. This review discusses subsequent clinical reports, relevant laboratory studies and topics for further research. RECENT FINDINGS Case reports have confirmed the efficacy of lipid resuscitation for local anesthetic systemic ...
متن کاملUse of lipid emulsion therapy in local anesthetic overdose
The use of intravenous lipid emulsion (ILE) therapy as antidote in systemic toxicity of certain agents has gained widespread support. There are increasing data suggesting use of ILE in reversing from local anesthetic-induced systemic toxicity severe, life-threatening cardiotoxicity, although findings are contradictory. Efficiency of ILE was demonstrated in animal studies in the treatment of sev...
